Information relevant to your skateboard activism Please make a copy of your completed application for your own records. A. Your Information The application must include the following information (1-13). Your information must be typewritten, in a legible 12 point type or larger.

High School Address (Street, City, State, Zip Code) 7. High School Telephone Number with Area Code 8. Will you graduate by June 2008? 9. High School Grade Point Average (on a 4.0 scale) 10. Name of College you plan to attend 11. Are you a United States Citizen? 12. Applicant's Date of Birth 13. Comment on your financial need for the award. (no more then one paragraph please) Essay must be typewritten, in a legible 12 point type or larger, double spaced, on a single page - approximately 300 words. Application must include your answer to the following question: How has skateboarding been a positive influence in your life? C. Letters of Recommendation Applicant must include two confidential letters of recommendation. One letter must be from a teacher or guidance counselor from your high school. The second letter must be from an adult who can attest to your dedication to skateboarding. D. High School Transcript Include - or request to have sent - an official high school transcript. E. Special Consideration for Skateboard Activism Students who have been actively promoting skateboarding in their community will be given special consideration. For example: Have you been active in getting a public skate park in your town? Have you created and circulated petitions, attended community meetings to lobby for a skate park? Have you published a letter to the editor of your town or high school newspaper in support of skateboarders? Initiated a fundraising drive for a skate park, held a fundraiser concert, skate jam or prize raffle? Have you started a website, newsletter, or school club that serves as a forum for skateboarders? Have you used your artistic talents to promote skateboarding? Please send documentation of these activities. You may send photocopies of any news articles pertaining to your fundraising efforts or activism, copies of published letter to the editor, or photos of your skateboard related activism. No items will be returned. Please do not send originals. The Patrick Kerr Skateboard Scholarship is awarded without regard to skateboarding skill. Do not send videos demonstrating skateboarding skill. |
